The objective of this evaluation form is to develop leaders on their level of inclusivity based on the 7 competencies identified. The results are used to drive additional training and coaching topics as well as analyze development over time.

Tessi developed the 7 key characteristics that embody an inclusive leader based on industry research from Deloitte, Harvard Business Review, etc. They are in order of internal to external progression:
- Awareness of Bias – Shows awareness of personal blind spots, as well as flaws in the system, and works to ensure a true meritocracy.
- Growth Mindset – Demonstrates an open mindset and deep curiosity about others, listens without judgment, and listens with empathy to understand those around them.
- Humility – Modest about capabilities, admits mistakes, and creates space for others to contribute.
- Empathy – Demonstrates ability to deeply understand other’s lived experiences & differences.
- Cultural Competence – Attentive to others’ cultures and differences, adapts to ensure everyone is and feels included.
- Effective Collaboration – Empowers others, pays attention to diversity of thinking and psychological safety, and focuses on team cohesion.
- Actionable Intent – Demonstrates authentic intentionality to diversity through actions and behaviors challenges the status quo, holds others accountable, and makes diversity and inclusion a personal priority.

You were chosen to provide your perspective in the individual’s level of inclusivity in the workplace based on your working history with them. The actions being evaluated are the actions observed during the entirety of your professional history with the individual.
Your quantitative and qualitative feedback is necessary to identify training needs & opportunities for improvement and to validate & strengthen the organization’s culture.

All responses are confidential. Results are reported in summary form only, with open-text comments (if applicable). Commentary will not be displayed in the report if there are less than 2 raters in the relationship group (Peer, Direct Report, etc.) and we ask that responses in the open text commentary do not contain individually identifying information.

The ideal response is comprehensive, specific, and constructive. Here is an example for commentary relating to Empathy from the perspective of a Direct Report:
“During our check-ins, Robin listens and takes time to get to know me as an individual. I had a personal issue that conflicted with my workload and Robin was able to arrange for accommodations without making me feel uncomfortable. One suggestion for Robin is to be more empathetic towards junior-level employees who may be struggling and looking for navigation with finding their place in the organization.”

To make the report objective and relevant to your professional performance, inviting evaluators who are personal friends and/or family members is prohibited. Evaluators must have a professional relationship and working history with you. Ideally, the longer the professional relationship, the better, as the person will have more insight about you.
Please ensure that you select a diverse group of raters who can provide meaningful feedback to ensure that you are receiving a true 360 perspective.
